If you choose to decline medical coverage, you must actively opt-out and select "no coverage" during the enrollment period.

Yes. If you enroll in the Hawaii Medical plan option for the first time, you will receive an ID card for your medical plan, including OptumRX.
The Options PPO Plan is a Preferred Provider Organization (PPO) plan. You can only contribute to a Health Savings Account if you are enrolled in a high-deductible health plan (HDHP), per IRS rules, so you are not eligible to contribute to the Health Savings Account.
However, if you enroll in the Options PPO plan or waive coverage, you can contribute to the Health Care Flexible Spending Account to pay for eligible health care expenses with pre-tax dollars. You can also contribute to the Dependent Care Flexible Spending Account to pay for eligible dependent care expenses.
If you have an HSA balance as of January 1, 2024, you can continue to use the funds at anytime to pay for qualified medical expenses, but you will not be eligible to contribute to the HSA as of January 1, 2024.
